Your first visit to our practice is the beginning of our therapeutic relationship. During your first visit, we ask our clients to provide some important background information (medical history) to better help us.

To ease waiting time, we have provided client forms below for you to download and bring with you during your first visit. Please take time to review the forms and also visit our “Meet Our Group” where you will find our counselors and their expertise.

If you are unable to download and complete the forms, please arrive a minimum of 15 minutes earlier than your scheduled appointment.

All payments, co-payments, and deductibles are due at the time of service.
